Skilled Care is offered in the hospital setting, with activities and weekly care conferences coordinated with the patient, family and care team present. Highly trained nursing staff observe and assess the condition of the patient during their stay, while offering the following services:

  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Diabetic teaching
  • Wound care
  • Ostomy care and teaching
  • Daily intravenous (IV) medications
  • Daily nutrition management and/or tube feeding
  • Pain management

For patients who may require additional medical care upon discharge from any hospital, Van Diest Medical Center offers post-acute services with our Skilled Care Program. This care is offered in our Inpatient Unit at the hospital, and is for those patients who may need additional therapy, services, or long-term antibiotics.

In a hospital setting, patients are able to receive more complex skilled care, allowing for better coordination of the care they receive from several disciplines throughout the hospital.

In order to qualify for Skilled Care Services for insurance purposes, a traditional Medicare patient would need a three-day inpatient hospital stay. Most managed Medicare and private or commercial insurance companies require a prior authorization approval.

Our Skilled Care Services are provided in a hospital setting, allowing for specialized care for the more complex patient and conditions. The program is overseen by a board certified Adult Gerontology Nurse Practitioner, with other medical professionals and providers onsite and accessible 24/7.

The Skilled Care Coordination Team includes a Social Worker and a Case Manager who is also a Registered Nurse. We have a quick referral acceptance into our program with weekend and evening admissions accepted.

The hospital’s Inpatient Department features large, private, family-focused patient rooms, spacious enough to provide overnight accommodations for family members and loved ones.

The Garden View Café, located in the heart of the hospital, is open daily for dining, serving delicious food options, with peaceful indoor and outdoor seating available.
