A satellite clinic of Van Diest Medical Center, Stratford Clinic opened in September 2011. Providing caring, friendly and patient-centered healthcare, Stratford Clinic cares for the whole family. The clinic is staffed by physician assistant Penny Osborn, PA, a certified medical assistant, and receptionist.
Patients are able to access a variety of services through the clinic, ranging from general practice and preventative health care needs to urgent and emergent health care services. Limited laboratory and X-ray services are available on site.

What is a Rural Health Clinic?
A Rural Health Clinic (RHC) can be a public, nonprofit, or for-profit healthcare facility with two main requirements needing to be met in order to be considered for RHC status. The facility must be located in a rural, underserved area and must use a collaborative team approach of physicians working with non-physician practitioners including nurse practitioners (NP), and physician assistants (PA), to provide services. The initial goal of the Rural Health Clinic (RHC) Services Act was to increase access to primary care services for Medicare and Medicaid patients in rural communities. The RHC program has remained remarkably consistent since the legislation was originally passed in 1977.
How does the Rural Health Clinic Accreditation Impact Patients of Van Diest Family Health Clinics?
For the majority of patients visiting Van Diest Family Health Clinics, there will not be a noticeable difference in the care received since receiving accreditation. However, there are a few changes with respect to specific medications being administered that affect some patients and where they will now receive these medications. Certain medications will no longer be administered in the clinic setting, but will now be available and administered in the Infusion Center located in the hospital setting. Please know the clinic and the infusion center staff are working together to make this process as efficient and seamless as possible for patients who may be affected by the change.
Additionally, patients who schedule an appointment for an annual wellness visit with the Health Coach will now speak with their provider during that visit as well. This allows for more comprehensive discussion regarding preventative health and wellness care for the patient.
Lastly, as Rural Health Clinics we are not able to accept and complete orders – such as lab, radiology, or medication orders – within the clinic setting from an independent provider who is not affiliated with Van Diest Family Health Clinics. These orders can, and will, still be completed; however, due to very specific RHC guidelines, those orders will now be done in the hospital setting rather than in the clinic. Again, clinic staff will work with hospital staff in the respective departments and the ordering provider in order to facilitate this process.
Why did Van Diest Family Health Clinics receive Rural Health Clinic Accreditation?
Van Diest Family Health Clinics in Webster City, Jewell and Stratford provide primary care services for Medicare and Medicaid patients, are located in provider shortage areas, and already utilize a team-based approach with physician and non-physician collaboration to provide services. An advantage of RHC accreditation is enhanced reimbursement rates for providing Medicare and Medicaid services. Quite often, the lack of healthcare professionals in rural areas may be due to the fact that rural communities are disproportionately dependent on Medicare and Medicaid as the principle payers for healthcare services and in the typical RHC, those payments can account for nearly 60 percent of practice revenue. Consequently, ensuring adequate Medicare and Medicaid reimbursement is essential to the availability of quality healthcare services in rural underserved areas.
