After we have taken care of your physical health, we want to take care of your financial concerns as well. Pre-Authorization
Your insurance plan may require notification for some hospital services. Please check your card, policy or carrier for their requirements. If you need any assistance with this, Van Diest Medical Center staff will be glad to assist you.
Insurance
Van Diest Medical Center will bill your insurance carrier based on the information provided by you at the time of registration. In the absence of necessary billing information, you will be responsible for payment of your entire account.
Van Diest Medical Center files your insurance claim as a courtesy service. However, you are ultimately responsible for paying the services provided.
Billing and Statements
Van Diest Medical Center will submit claims for the following services:
- Hospital charges:
- Emergency Room
- Inpatient and Observation stays
- Surgical services
- Laboratory
- Radiology
- Infusion
- Clinic charges for Van Diest Family Health Clinics of Webster City, Jewell, and Stratford
- Anesthesiologist/CRNA
- Emergency Room Physicians
- General Surgeon
- Specialty Clinic:
- Pain clinic
- Cardiology clinic
- Physical Therapy and Occupational Therapy
Please Note: Physical Therapy and Occupational Therapy services are provided by 21st Century Rehab but are billed under Van Diest Medical Center
You may also receive bills from other providers for related services, such as Radiologists, Pathologists and Physicians. These groups will bill you directly for services they provided.
Non-Covered Services
Insurance companies do not pay for all medical services, even those that might be helpful to a patient. When a service is not covered by your insurance policy, you are responsible for paying the bill.
Physicians dictate your diagnosis for each visit or procedure. We are unable to change this information just so the claim will be paid.
Third Party Liability
If you have been injured in some manner, whether through an automobile accident, fall, etc., Van Diest Medical Center will bill the liability carrier you specify, followed by your health insurance if any patient responsibility remains.
Although another party may have some legal responsibility for your injuries, this matter is between you, your attorney and the party responsible for the injury. As the patient, you may be responsible for paying the bill if liability is not settled within 90 days.
If you are a Medicare or Medicaid recipient, health care laws require that we bill the liability carrier first, then Medicare or Medicaid.
If you prefer to file the claim directly with the insurance company, please contact the Business Office at 515-832-7715 so we can provide the information for you to do so. We ask that you please provide us with the insurance contact information so we may follow up on payment

It is the responsibility of the individual to verify that his/her insurance plan will cover medical services provided by Van Diest Medical Center. Please verify with your insurance company prior to having services.
Please present your insurance cards each time you visit to ensure prompt and accurate filing of your insurance.
Your insurance company may deny or hold claims pending information for such reasons as possible third party liability, possible work-related, or coordination of benefits. Please respond to all requests for information from your insurance company in a timely manner.
