This position will serve as the inpatient Social Worker responsible for assisting with the assessment of the psychosocial and discharge planning needs of patients admitted to Van Diest Medical Center. In addition, this position will support the Emergency Room, OB and other areas of the hospital as needed.
Wage commensurate with experience.
A bachelor’s degree or master’s degree in Social Work, Human Services or Psychology and/or three or more years of experience in a healthcare setting.
License in Social Work preferred
Assess patient needs, situations, and support networks to assist with achieving their healthcare goals
Participates in multidisciplinary rounds on the inpatient and skilled units
Coordinates and plans programs and activities to meet patient/family social and emotional needs
Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle
Assists and coordinates discharge planning for patients
Refers consumers to appropriate resources and community agencies as indicated and completes follow up to ensure they are effective
Assures maintenance of patient records in strict compliance with policy, regulation and procedure
Assist providers and other healthcare professionals with understanding the effects that diseases and illnesses have on patients’ mental and
Familiar with standard concepts, practices, and procedures within the hospital setting
Represents Van Diest Medical Center on community committees as applicable
Adheres to standard precautions, exposure control plan, hospital safety and infection control policies
Participates in developing, revises and reviewing policies as applicable
